OWNER'S ACCEPTANCE: I hereby accept the terms contained on these pages and authorize the licensed contractor (ROC 358383) to complete the work as detailed in this Contract. I agree to pay in accordance with the payment terms above, and declare that I have read and understand the Terms & Conditions in this document.
1. Materials arrive to jobsite. 2. Dumpster supplied by contractor. 3. Tear-off existing roof down to decking. 4. Replace any damaged wood decking.
Asphalt
5. Install drip edge and rake edge metal. 6. Install new pipe jacks and boots. 7. Install GAF Weather Watch Leak Barrier on valleys, around skylights, chimney, and all penetrations. 8. Underlayment over roof deck: GAF Tiger Paw Synthetic Underlayment. 9. Install GAF PRO START Starter Strips along all eaves and rakes. 10. Install GAF Dimensional Shingle (as selected above) using 1¼ inch roofing nails (nails may be visible in attic and under soffits) per GAF wind warranty specifications. 11. Install new GAF ventilation components (if necessary). 12. Cap ridges and hips with GAF Z-Ridge Cap Shingles. 13. Clean area and remove all job-related debris (exceptions include any debris in attic space as a result of ventilation replacement and/or addition).

5. Install new drip edge metal. 6. Install underlayment (per system selected). 7. Install battens. 8. Install Westlake-Boral ventilation system components (on applicable systems). 9. Install tile (full replacement or remove & reuse existing, per selection). 10. Ridge/hip tile placement (Solite). 11. Clean area and remove all job-related debris (exceptions include any debris in attic space as a result of ventilation replacement or addition).
Any decking needed in addition to the specified number of sheets of plywood will cost an additional $100.00 per sheet. Remove & Reuse Tile projects include limited use of new tiles at the contractor's discretion; if a new tile is used, color may not match existing tiles.

We are general contractors who specialize in roofing — no painting and no stucco work.
Items attached to the house or roof (lights, satellite, etc.) can break when removed — we'll take them down if asked, but we're not responsible for breakage. If you're concerned, remove them yourself or have a handyman do it before we start.
Satellite dish: we will reinstall it, but we are not responsible for signal — please call your satellite carrier.
Please remove valuables from your walls — expect noise and shaking of the house.
Take down anything breakable from cabinets, stands, or hanging displays to prevent breakage.
Expect heavy noise from start to finish — this is an active construction site.
For your safety, please do not get on the roof during the project.
Please do not direct the installation crew — they do not need help.
A project manager is on-site or stops by the job once a day.

Jobs are scheduled in 5-day windows; if the crew misses a day, no need to worry — we have 5 days to complete.

Your yard will be messy throughout; we do daily cleanup with a full cleanup on the last day.

The agreement on the face hereof and any agreement made pursuant thereto between Contractor and Owner(s) will be subject to all appropriate laws, regulations, and ordinances, and to the following special terms and conditions:
1. FINAL APPROVAL. All proposals are subject to approval by Contractor's Credit Department and Management.
2. HOA RULES. It is customer's responsibility to ensure that the materials selected and installed are compliant with any applicable Homeowners' Association rules; Contractor shall be entitled to a change order for any additional Work required as a result of non-compliance.
3. CONTRACT PRICE. The Contract Price shall be the initial Contract Price plus the cost of upgrades and additional Work requested by Customer, if any. Any price stated is an approximation made in good faith based on currently available information and initial visual observation and is subject to change. Replacement of deteriorated roof decking or other materials, unless otherwise stated, will be replaced at additional cost over and above Contract Price. All extra costs required from code upgrades will be the responsibility of the Customer.
4. PROJECT CONTRACT. Customer represents and warrants that Customer is the Owner(s) or an authorized representative of the Owner(s) of the above property and has the financial strength and ability to pay in full for the Work. Customer is responsible for payment for all labor and materials furnished.
5. WARRANTY. Contractor will provide a five-year workmanship warranty on residential Tile, Flat, and Asphalt installation. Goods furnished are products of reputable manufacturers; Contractor will use best efforts to obtain manufacturer warranty documentation upon request and shall not be responsible for product defects. THE WARRANTY IN THIS PARAGRAPH IS THE SOLE AND EXCLUSIVE LIABILITY OF CONTRACTOR; EXCEPT AS TO TITLE, THERE ARE NO OTHER WARRANTIES, WRITTEN, ORAL, IMPLIED OR STATUTORY. NO IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE SHALL APPLY. No warranty shall attach until Contractor is paid in full and is not transferable. No warranty obligation exists for defects caused by: (i) normal wear and tear; (ii) Customer's gross negligence or willful misconduct; (iii) vandalism or acts of third parties not under Contractor's control; or (iv) events beyond Contractor's control, including acts of God and extreme weather.
6. PERFORMANCE. Customer shall provide access to the premises. All Work shall be completed in a workmanlike manner per generally accepted construction guidelines and local codes in effect at the time of this Contract. Contractor may utilize subcontractors at its discretion. Contractor shall not be liable for delays or damages caused by third parties, strikes, or material/labor shortages beyond its control. If unusual conditions are discovered requiring a change in Work, Contractor may increase the price as required and shall notify Customer. Customer carries risk of loss for all Work/materials delivered or installed. All surplus materials belong to Contractor.
7. ELECTRONIC CONSENT. By providing a mobile number, Customer expressly consents to receive text messages and calls from Contractor and its affiliates, agents, and independent contractors, including informational, transactional, and promotional notices. Message and data rates may apply. Customer may opt out by replying "STOP."
8. CANCELLATION. Customer agrees to pay Contractor for its services at a rate of $120.00 per hour or 10% of the Contract Price, whichever is greater, as a Cancellation Fee if Customer cancels after the 3-day Right to Cancel elapses and before Work commences. Customer shall be responsible for direct and indirect costs incurred. Customer agrees not to engage any other service provider for work connected to the scope of this Contract without Contractor's written consent.
9. GUARANTEES. Contractor guarantees all materials as specified and that the Work shall be completed in a workmanlike manner per standard practices.
10. FORCE MAJEURE. This Contract is contingent on the nonoccurrence of events beyond Contractor's control, including acts of God, extreme weather, labor strikes, accidents, or delays in material delivery.

11. INSURANCE. Customer agrees to carry fire, tornado, wind, hail, water damage, and other necessary insurance.
12. INTEGRATION. All sales are expressly conditioned on Customer's agreement to these terms. This Contract represents the entire agreement between the parties; there are no oral or written understandings that add to or modify it. No modification shall be valid unless in a signed writing. This Contract is subject to final approval by Contractor, who may assign its rights and obligations in its sole discretion.
13. SEVERABILITY. If any portion of this Contract is held invalid or unenforceable, the validity of the remaining provisions shall not be affected.
14. JURISDICTION. All issues and disputes shall be governed by the law and venue of any competent jurisdiction in which the Contractor maintains an office, at Contractor's sole discretion. CUSTOMER AND CONTRACTOR KNOWINGLY, VOLUNTARILY, AND IRREVOCABLY WAIVE, TO THE FULLEST EXTENT PERMITTED BY LAW, ANY RIGHT TO A TRIAL BY JURY IN ANY LITIGATION ARISING OUT OF THIS CONTRACT.
15. PAYMENT / REMEDIES. Customer agrees to pay all amounts when due. Delayed payments accrue interest at 18% per annum (or the highest legal rate, whichever is lower), plus a $50.00 returned check charge. Customer's remedies for any breach are limited to replacement of merchandise or services rendered. IN NO EVENT MAY CUSTOMER RECOVER DAMAGES GREATER THAN THE PRICE ACTUALLY PAID, NOR SHALL CONTRACTOR BE LIABLE FOR ANY INCIDENTAL, SPECIAL, INDIRECT, EXEMPLARY, PUNITIVE, OR CONSEQUENTIAL DAMAGES. If payment is more than thirty days past due, any discount or promotional price shall be void.
Customer authorizes Contractor to use photographs and/or recordings of the project for marketing purposes; Contractor retains copyright and Customer waives claims for compensation unless a written revocation is submitted prior to publication via certified mail.
The Contractor will not file any insurance claim on behalf of the consumer related to the work performed under this contract; the consumer agrees to handle all insurance claims directly with their insurance company.

This notice is sent pursuant to Arizona law to inform you that the licensed contractor (ROC 358383) has furnished or will furnish labor, materials, or professional services in connection with the improvement of your property. If bills are not paid in full, a mechanic's lien leading to foreclosure proceedings may be placed against the property. You may protect yourself by requiring a conditional or unconditional waiver and release pursuant to A.R.S. §33-1008, or any other appropriate method.
